Errors - VoiceAIWrapper

When a tool can’t do something, it returns a tool error (isError: true) with one sentence saying what happened and what to do next. The AI app usually passes it on in its own words.

When a sign-in error happens, the error also carries an OAuth challenge, so AI apps that support it show a Connect button straight away.
